Please complete by filling the order form in following steps wich are necessary for order identification and the right sending of additional requested material test reports to the receiver.

Complete return address and contact name

2.  Data for request

2.1 Safety valve data

All available technical product data

2.2 LESER order

Data as like LESER Job-No. or Serial-No. are to decide in the available sales documents

3. Selection and markiing

Mark the component unit in the columns A+B

In column C depending upon component unit mark heat-no. or code-no. and indicate the material right under it

To locate the heat-no. or code-no. of component unit click in column D the displayed link

4.  Order reference

Available material test reports are to be selected acc. to LESER product catalogue' s "How to order"

5.  Special request

Inquiries as for example of welded flanges the welding test and X-ray test, declaration of FDA conformity for rubber materials or tightness test can be requested at present only with this form. Please state in writing.
